What to Avoid

Worn-out shoes. Cushioning breaks down with use, even a well-designed shoe loses its protective qualities over time. Completely flat, unsupportive sandals and flip-flops. These offer essentially no structure and typically no ability to add a supportive insole, a poor choice for extended wear if you’re managing ball of foot pain.
Minimalist or “barefoot-style” shoes, at least without an extended, gradual transition period. These intentionally reduce structure and cushioning, which is precisely what someone with metatarsalgia typically needs more of, not less.
The Insole Still Matters, Even With the Right Shoe
A well-chosen shoe creates the right conditions, room, cushioning, appropriate stiffness, but a proper metatarsal support insole is what actually redistributes pressure away from the painful area in a targeted way.
